51 Year Old Dies After Getting Caught Between Equipment

The death of a 51 year old chemical technician at a coatings company in Mosinee, WI has prompted a federal investigation which had resulted in multiple safety violations.

OSHA issued three repeated, four serious, and three other than serious safety citations to Crystal Finishing Systems follow the employees death on June 14, 2016.

The worker died when an automated crane pinned him between the crane hook and dip tank load bars as it was moving product from one tank to another.  He was pronounced deceased at the scene.

Crystal Finishing Systems failed to:

  • Adequately guard machines to prevent workers from coming in contact with operating parts.
  • Protect workers walking working surfaces.
  • Provided adequate personal protective equipment.
  • Train workers about hazardous chemicals in use at the facility.

OSHA has proposed penalties of $171,169.  The company has 15 days from receipt to appeal or comply with the violations.

We see this all too often that a person goes out to work to make a living for their family and do not make it home at night.  It is a shame that companies knowingly fail to protect their employees, and even worse when a tragedy like this occurs.
