Henry Mathusek of Naperville, IL was arrested and charged by the Bergen County Prosecutor for insurance fraud and forgery related crimes. The arrest comes after an investigation conducted by the Bergen County Financial Crimes Unit.
In August of 2016, the Bergen County Prosecutor’s Office received a complaint of insurance fraud filed by The Hartford insurance company against Henry Jared Masthusek, the owner / operator of Bison Floors in Moonachie, New Jersey. In the complaint, The Hartford representatives alleged that Mr. Mathusek had forged insurance certificates in an effort to create fraudulent proof of work comp insurance while still conducting business and applying for contracts.
An investigation by members of the Bergen County Prosecutor’s Office Financial Crimes Unit revealed that Mr. Mathusek had used a computer PDF editor to forge and alter Certificates of Insurance which he submitted as proof of insurance while working at job sites and applying for valuable contracts. The insurance certificates purported to provide proof of a Hartford Worker’s Compensation Policy which Bison Floors did not have at the time of submission.
Henry Mathusek is scheduled to appear in Bergen County Central Judicial Processing Court for a first appearance on January 25, 2017 at 8:30 a.m.
Prosecutor Grewal states that these charges are merely accusations and that the defendant is presumed innocent until and unless proven guilty beyond a reasonable doubt.
We have definitely reported on our fair share of fraudulent activity in the work comp community over the years. This is definitely one of the first stories that I have written about where someone went as far as to use something like an Adobe PDF editor in order to mock up fake certificates of insurance. I will do my best to keep up on the results of this story and see where it leads.
