The United States Department of Labor’s Occupational Safety and Health Administration (OSHA) cited an Illinois manufacturer for 18 different health and safety violations at their Frankfort, IL facility.
The company, HB Fuller operating as Adhesive Systems Inc., received a serious violation for failing to have energy control procedures in place while employees cleaned mixers- exposing them to struck by and amputation hazards. They received another serious violation because their energy control procedures did not outline the techniques to be used to control various energy sources, and they failed to adequately train employees on proper energy control procedures which exposed them to amputation and moving machinery hazards. They failed to provide employees with respirators appropriate for the hazardous atmosphere.
They received other violations for failing to ensure markings and nameplates were in place, failing to ensure that employees operating industrial trucks were trained to do so, failing to ensure installed equipment was intrinsically safe, failing to implement an emergency response plan, and failing to complete personal protective equipment assessments which put employees at risk of chemical exposure. The proposed penalties total $587,564.
