An Ohio-based grain handling company was fined by the Occupational Safety and Health Administration after alleged safety violations led to the deaths of two employees last year.

The Andersons Inc. was fined by OSHA for $291,716 after the administration found grain handling, walking and working surfaces violations related to the July 19th accident.

The company is in Monclava Township, OH. Two of their employees suffocated after becoming trapped in a grain storage tank.

The agency cited the company for two willful and two serious violations for not developing an emergency action plan that included the proper procedures for grain rescue and coordination with local rescue services.

The company said they have cooperated with OSHA throughout their investigation and are shaken by their tragedy.
