The United States Department of Labor’s Occupational Safety and Health Administration (OSHA) cited Arbre Group Holding, doing business as Holli-Pac Inc., for willful and serious safety and health violations at their Holley, NY facility. The company is a frozen food packager for retailers.
Inspectors found that the company exposed workers to laceration and amputation hazards related to a package filler machine. Parts of the machine did not have doors to prevent employees from contacting operating parts, and the access door on another part of the machine lacked an interlock mechanism to stop the machine’s operation when the door opened.
They were also cited for inadequate lockout/tagout procedures and for failing to train employees on hazardous chemicals in the workplace, obtain annual audiograms for employees exposed to excessive noise, ensure employees were wearing appropriate eye and face protection and for failing to repair damaged parts on electrical equipment. They face $200,791 in penalties.
The inspection was conducted under the Site-Specific Targeting Program. This program directs enforcement resources to workplaces where the highest rates of injuries and illnesses have occurred.
